Description: This book approaches diagnosing from a specific angle, that of starting with symptoms and narrowing the differential from the symptoms. The previous edition published in 2013.
Purpose: The premise is to maximize the information from the history elicited from the patient before turning to the physical examination and ancillary testing. This is a very reasonable and classical approach, as the information obtained during the historical interview has been the traditional approach, but it has been somewhat superseded by ancillary testing.
Audience: The audience is every clinician who has the opportunity to care for patients in their everyday practice.
Features: The book is divided into three parts: getting the most out of your history and physical examination; algorithms of symptoms and signs; and diseases that are symptoms of other diseases. These are followed by two appendixes with suggested orders for the workup of common symptoms and the laboratory workup of disease. The chapters in part 2 on symptoms start with a case history and a series of questions that help define the problem. This is followed by a section of the diagnostic workup with tables and algorithms to follow. The chapters dealing with abnormal laboratory test results or isolated physical signs found during an examination start with suggested questions to narrow the focus of the etiology along with an algorithm to follow.
Assessment: This is an interesting book for inexperienced clinicians to hone their critical thinking skills and it has great value for experienced clinicians. The inability to access the electronic version is a significant impediment, as this would be an excellent resource to have on a smartphone for consultation between patients.
