Overview

The book provides an up-to-date account of the neuropsychological, cognitive-neurological, and neuropsychiatric aspects of movement disorders. The past ten years have seen an explosion of research covering non-motor aspects of Parkinson's disease and, more recently, movement disorders such as essential tremor, dystonia, corticobasal syndrome, progressive supranuclear palsy, and multiple system atrophy. It is often these neurobehavioral features that become troubling to the patient: they are sometimes difficult to recognize and treat, are associated with diminished patient and caregiver quality of life, and may hasten disease progression, loss of independence, and institutionalization. This book discusses the most recent diagnostic and treatment guidelines for such cognitive and psychiatric conditions in Parkinson's disease and other movement disorders, while providing practical tips and strategies for general assessment.

About the Author

Dr. Alexander Tröster's clinical and research interests include movement disorders, cognition, and deep brain stimulation. He has authored more than 180 scientific articles, medical journal articles, and book chapters, and his contributions have been recognized with fellowships from the National Academy of Neuropsychology and the American Psychological Association. A past president of the National Academy of Neuropsychology, he is the recipient of their Distinguished Scientific Contributions to Clinical Neuropsychology and Early Career Achievement awards. He serves on the editorial boards of five journals and several advisory boards.

Description: There is a penchant among neuropsychologists for writing about Parkinson's disease, but much of the time it is within the context of neurodegenerative diseases. This book takes a specific look at movement disorders and their associated cognitive symptoms.
Purpose: The aim is to update the literature over the last decade on the neuropsychological findings associated with Parkinson's disease and other movement disorders, as well as to provide a resource for the latest diagnostic and treatment guidelines.
Audience: This is appropriate for clinical neuropsychologists and neurologists working with patients who have movement disorders, as well as trainees in those disciplines. It could be of interest to allied paraprofessionals involved in rehabilitation, such as speech and occupational therapists. However, it is written at a rather basic level that may be tedious for experienced clinicians. The editor is well known in the field and many international authors contribute, but they are not always household names.
Features: The book begins with an overview of the key brain circuits involved in movement disorders, which is a detailed, but educational review of neuroanatomy, followed by a chapter on screening cognitive deficits. Although it reviews several screening measures, the coverage of the strengths and weaknesses is inadequate for some, missing key psychometric articles. This chapter also fails to fully consider the range of functional assessment instruments, instead essentially stating that there are none. The next chapter on more comprehensive assessment has similar shortcomings, covering the usual tests taught long ago in graduate school without any critical review of their psychometric properties, which recent studies have questioned for some of them. Other chapters address this much better, such as that on quality of life measures. The remainder of the book has reasonable coverage of the topics, but it seems somewhat light on neuropsychological studies and clinical information for a book with the focus suggested by the title. For example, there are no case studies, data summaries, or other immediately clinically useful nuggets. There is very little information regarding differential diagnosis approaches or algorithms and, although it is purported to provide diagnostic guidelines, some chapters never even mention these. Additionally, there is an overall dearth of images, illustrations, tables, or figures. The few that show up in the neuroimaging chapters are small and of fairly poor quality.
Assessment: At a basic level, this book provides a rudimentary understanding of movement disorders and would be a good starting place for trainees. However, it falls well short of providing an advanced look at the assessment of movement disorders, and fails to deliver a resource of ostensible practical value to clinicians.
