Description: This is a quick-reference pocket guide for nurse practitioners (NPs) who practice in the emergent/urgent care setting. The guide takes the practitioner step by step through a patient encounter including documentation, physical examination, history taking, and medical decision making (MDM) in the emergent/urgent care setting. The second edition has updated clinical guidelines, additional illustrations, updated ultrasonography information, and updated pharmacology information. The guide is structured in a head-to-toe approach based on the most frequently encountered presenting complaints. Documentation standards and templates are discussed in the first section, and the other sections of the handbook describe numerous emergent/urgent presentation types. The guide is intended to assist the provider with paperwork while also ensuring that all necessary elements are addressed. The main content is in bullet points, but each section includes a brief paragraph summarizing the MDM as well as possible differential diagnoses, which are written in bold. The majority of the sections close with a section on "tips" and another one on "don't miss." Although concise, the amount of information aimed to enable the practitioner through a patient encounter is priceless.
Purpose: The purpose is to serve as a handy reference manual. The authors accomplished this goal by structuring the text systematically and succinctly. The information is valuable and would assist any clinician who works in an emergent/urgent care setting. The documentation templates provide a structured approach to patient care and medical decision making.
Audience: This book is designed for nurse practitioners who work in the emergent/urgent care setting. However, this guide would benefit various practitioners, including physicians. The templates for documentation and the systematic approach to establishing an MDM are priceless. I believe it would be an excellent resource for student nurse practitioners as well. The guide would assist students in developing a strategy for documenting, assessing, developing a treatment plan, documenting MDM, and generating a differential diagnosis list from which to work. Authors Shea and Hoyt are both highly qualified professionals with extensive experience in emergency care. Dr. Hoyt is an emergency nurse practitioner and the director of the NP/ENP program at the University of San Diego. Ms. Shea is an accomplished ENP with more than 40 years of experience. Shea established a clinical fellowship for APPs at St. Mary Medical Center in Long Beach.
Features: This book is a concise pocket reference for nurse practitioners who practice in the emergent or urgent care setting. However, the guide would be an excellent resource for any emergency provider, novice or experienced, including physicians. The guidebook takes a step-by-step approach to documenting a patient contact in the emergent/urgent care setting, including history taking, physical examination, and medical decision making. In following this approach, providers will ensure that all documentation requirements are met while also ensuring that the patient is assessed and treated according to the most current guidelines. Additionally, the templates for procedural documentation are noteworthy and are an often-overlooked resource when compared with additional guides or reference material. Finally, the book is organized in a head-to-toe format to facilitate quick retrieval of information about the most often encountered presenting problems. The cons are similar to those of many other printed publications: out-of-date terminology, diversity concerns, current EBP management, and keeping current with new diagnoses. For example, COVID -19 is not mentioned and is currently one of the top diagnoses seen in the emergency setting. Another example: the term "hypertensive urgency" is no longer recommended. As for diversity inclusion, I would like to see the authors be sensitive to cultural and ethical differences. For instance, skin charting templates include the abbreviation "PWD," which stands for pink, warm, and dry. Not everyone has "pink" skin. Another problem is the mistaken belief that anyone who utilizes this guidebook has an "attending." The guidebook implies that an MD provides attending or consultation services. This is not always the case, though. Numerous NPs practice without the supervision of a physician. More importantly, I believe the NP profession has to acknowledge that we are capable of consulting with a wide variety of healthcare professions without being bound by the age-old adage that it must be a physician or "MD." Overall, the book is an invaluable resource for any emergency clinician. The "tips" and "do not miss" sections add value to an already outstanding resource. I would like to see a copy of this book in the hands of every ENP student.
Assessment: This book is an excellent, well-organized pocket guide written for nurse practitioners practicing in the emergent or urgent care setting. While there are many emergency care educational references, this book focuses on common patient complaints and the associated history taking, assessment, and management, while ensuring the appropriate key elements are documented. The majority of emergency textbooks fail to address the importance of documentation. The second edition adds to the first edition with updated clinical guidelines, additional illustrations, updated ultrasonography information, and updated pharmacology information.
