Description: This 19th edition is a classic for those working with clients who are on or need to be prescribed medication while breastfeeding. The book is updated approximately every two years to include new medications as well as new studies. The preface is an important aspect as it reviews the medication transference physiology into the breastmilk, presents general suggestions for the clinician, and then ends with how to use this resource. The How to Use this Resource section explains the medication monograph: the ability of the drug to enter milk, concentration, timing, and several other parameters important for the clinician to know before prescribing.
Purpose: The purpose of this book is to improve clinicians' safe prescribing practices to breastfeeding clients while also not recommending discontinuation of the best practice of breastfeeding without solid evidence. These are worthy objectives and due to the frequency of new medications, frequent updates are necessary to the book. These objectives are met with the addition of new medications, updated studies, and subsequently updated recommendations.
Audience: This book is written for students, residents, practitioners, and possibly clients who need evidence-based information on medications' breastfeeding safety profiles. It is appropriate for and meets the needs of any provider who is caring for a breastfeeding client and would not be limited to only maternal/child health providers as it covers most medication classifications. Dr. Hale has been a published expert in this field for over 20 years and this book is a standard for any maternal/child practitioner.
Features: This book is organized by medication name from A to Z. Each medication has four sections: 1. Trade name, medication category, and lactation risk rating; 2. Information about the compatibility of the drug with breastfeeding and study findings; 3. Monogram with six criteria which are explained clearly in the preface; and 4. Adult and pediatric concerns, dosing, monitoring information, as well as references. The scientific writing is clear yet concise and the standardized formatting and highlighted areas (name(s) and monogram) direct the clinician while the prose gives supportive information. The only shortcoming is the organizational method. It would be helpful to have some type of cross-referencing appendix organized by medication classification instead of just by medication name.
Assessment: This is a high-quality, standard text for any maternal/child clinician. Its scientific writing is meant for providers although the clear lactation safety rating could be used by clients to guide their engagement in medication discussions with their providers. There are some books intended for the client such as Breastfeeding and Medication, 2nd edition, Jones (Routledge, 2018), which contain less scientific language and focus more on the commonly prescribed medications for women - but for providers, this is the gold standard. The regularly updated editions are a necessity as new medications and new data are discovered.
