Description: This is the third edition of a very popular handbook of kidney transplantation that first appeared in 1992. Over the years, the book has by necessity become thicker, but it still fits comfortably in the white coat during ward rounds.
Purpose: This book is intended to be a constant companion to the clinician taking care of the renal transplant recipient and remains a handy reference for clinical problems faced in day-to-day practice. In this role, it more than meets its objective.
Audience: It is also suitable for students, nurse coordinators, and residents in surgery, medicine, and nephrology. The field of transplantation and immunosuppression is rapidly changing and an updated version is a great necessity.
Features: New immunosuppressive drugs, their use in specialized situations, and other recent trends are well set out. Since this is one of the most rapidly advancing areas of transplantation, the chapters on transplantation immunobiology and immunosuppressive drugs have been revisited and updated. With the expansion of donor organs to include those with less than ideal function newer techniques and surgical cautions are necessary as well as change in immunosuppressive protocols to minimize complications. These are discussed elegantly. The chapter on long-term care of the kidney transplant recipient has been completely revamped. Although there is a listing of the principles of treatment, a pragmatic approach has been suggested for the avoidance of common pitfalls. I particularly liked this approach, which simplifies the complex interactions as much as possible. The new chapter on hepatitis reflecting the increasing problem in this population brings new algorithms for managing these patients. The chapter on diagnostic radiology in kidney transplant is well illustrated and reflective of the current management of patients.
Assessment: The chapters are written by very experienced people, mostly from the U.S., giving it great credibility but with a decidedly California flavor. This handbook set a high standard with its first two editions and has managed to raise it further. Its strength lies in the fact that it is up-to-date, lucid and clinically relevant.
