Overview

Poverty, substandard medical care, social neglect or withdrawal, unhealthy lifestyle—these are just some of the contributors to the substantial morbidity of patients with severe mental illness. Medical deteriorations are often unexpected and severe, and particularly difficult to evaluate in the context of psychotic disorders.

For this new edition, the Handbook of Medicine in Psychiatry has been updated and streamlined to provide a realistic approach to the medical issues encountered in psychiatric practice by helping clinicians answer whether their patient: • Is at risk of dying or becoming severely disabled.• Requires an immediate therapeutic intervention for a potentially life-threatening condition.• Needs to be transferred to an emergency medicine setting.• Requires urgent investigations.• Must have changes made in the current medication regimen. Clinical vignettes for each chapter illustrate the complexity of the presentation of abnormal vital signs and somatic disorders in psychiatric settings, including fever, hypertension, seizures, and nausea and vomiting.

The guide also provides risk stratification for major complications—from abnormal thyroid function and acute kidney injury to myocarditis and venous thromboembolism—enabling readers to determine the need for a transfer of the patient to an emergency medicine setting.

A brand-new section features thorough discussions of topics requiring interdisciplinary collaboration with geriatricians, neurologists, anesthesiologists, addiction medicine, and adolescent medicine specialists.

Clinicians working in today's busy inpatient and outpatient psychiatric settings will find in these pages a cognitive framework and knowledge base that will aid them in accurate decision making in the conditions of uncertainty created by potentially major medical deteriorations of the vulnerable populations under their care.

What People are Saying About This

Javier I Escobar M.D.

This is the third edition of a seminal book first published in 2006. The first author, Peter Manu, a distinguished internal medicine specialist, has been imbedded in psychiatric programs as a key faculty member for several decades, actively participating in clinical and research activities. Professor Manu is also a leading national and international expert on functional somatic syndromes. The two other editors are psychiatrists, also based at Long island Jewish. The book includes 33 chapters, each chapter focusing on a specific medical issue that is quite relevant for patients suffering from mental disorders and particularly to professionals working in mental health settings. Each of these chapters has been neatly crafted and prepared by experts, and elegantly framed by the editors within a succinct, user-friendly, colorful format that includes a brief illustrative clinical vignette at the beginning of each chapter. The book nicely reflects a "commonsense approach" to these medical situations. It is clearly written and includes a list of essential references. The book is an essential addition to the work desks of practicing psychiatrists, psychologists and other mental health professionals.

Douglas M. Ziedonis

Drs. Manu and Karlin-Zysman have made an outstanding and important contribution to the field. This is a great resource and guide for psychiatrists and other health care providers in better assessing and treating comorbid medical conditions. The Handbook is very comprehensive, accessessible, and practical. Complex diagnostic profiles are well elucidated and the integrated treatment approaches iinformative.

Frank Schneider

The second edition of the Handbook of Medicine in Psychiatry is a remarkable advancement of the already highly appreciated first edition. The book is even more a valuable source of sound information for students and practitioners alike. Newly introduced chapters address highly important issues, such as sudden death in psychiatric settings and the structure of the book has been rearranged in a more pleasingly way, which supports the understanding of different aspects of common syndromes, such as metabolic emergencies. Overall, this book is a comprehensive, valuable and practical guide to physical symptoms for psychiatrists and physicians from all other disciplines.

Povl Munk-Jørgensen

The second edition of Handbook of Medicine in Psychiatry edited by Peter Manu and Corey Karlin-Sysman is in fact more a re-writing than an edition. Several new and highly skilled authors have joined the group and a useful grouping of chapters has been introduced. The handbook's obvious first priority is usability for the clinical psychiatrist. As it is further thoroughly updated and easily used because of a very specific build up of chapters this handbook will shortly be found on the shelves in every psychiatric department and clinic next to the diagnostic manuals where it can be reached by any psychiatrist, physician, medical student and nursing staff dealing with psychiatric patients. The book makes it totally clear to the reader that a psychiatrist must be a physician first, secondly a psychiatrist just as the book documents that psychiatry is truly holistic - mental illness is not only mind and brain but also soma. The pronounced excess mortality from physical diseases in mentally ill through many decades is the biggest scandal in psychiatry since the era of the backward. This book offers the practical tools to overcome this.
