Purpose: The purpose is to provide an overview of the need for continuity of care and involving patients in their care in the community. It is an interesting book, but not one necessary for the care of patients in the community.
Audience: The book appears to be written for those practicing in the field of community health and on the collaborative relationship between the members of this population of study.
Features: The book is meant to discuss the relationship between healthcare providers and the populations they treat. It discusses the interaction and collaboration necessary to provide patients a holistic approach to care in a community setting. The book is interesting, but does not present any specific innovative ideas for capturing the patient at this care level.
