Overview

Emerging as a new sub-specialization within the hospitalist community, the neurosurgery hospitalist provides preoperative risk stratification, advises on managing pre- and postoperative complications, and helps doctors make decisions about when to involve specialists other than neurosurgeons. This collaborative approach to the neurosurgery patient has been shown to offer effective care since hospitalists can be better attuned than specialists to multiple medical problems that most patients have.
Medical Management of Neurosurgical Patients is a first of its kind textbook providing a standardized source of information for neurosurgery hospitalists in order to establish a common ground and improve their knowledge and training. The work will focus on management of CNS infections, management of bleeding in the context of CNS surgery (a potentially catastrophic complication), management of sodium and blood glucose levels including steroid-induced hyperglycemia, perioperative pain control, and management of pressure injuries and rehabilitation in the context of CNS injury.

About the Author

Dr. Daniel is an attending physician taking care of neurosurgery and orthopaedic spine surgery patients at the Thomas Jefferson University Hospital and the Jefferson Hospital for Neuroscience. He also serves as the Lead Farber Hospitalist at the Jefferson Gibbon Building and leads his own laboratory at the Center for Translational Medicine of the Department of Medicine. His research aims to develop and test innovative methods to prevent HIV-1 infection, with a particular focus on women's health. He lives in Ambler, Pennsylvania with his wife and two children.
Dr. Harrop serves Thomas Jefferson University as their Division Director of Hospital Medicine for the Vickie and Jack Farber Institute of Neuroscience. In collaboration with 14 other Hospitalists, Dr. Harrop provides co-management services for Thomas Jefferson University's Department of Neurosurgery. Dr. Harrop lives outside of Philadelphia with her Husband and two children.

Description: This short book is a concise overview of medical problems and possible surgical complications to consider when taking care of neurosurgical patients, a unique population that requires comprehensive care that extends beyond just the nervous system.
Purpose: The aim is to provide a broad overview of the medical considerations in patients with neurological pathologies that may require care after surgical intervention. It concisely covers topics such as preoperative evaluation, antiepileptic medication, sodium disorders, fever, and pain management.
Audience: This book provides a solid foundation for junior level residents and midlevel providers to build upon as they become more experienced in their care of neurosurgical patients in the critical care and ward settings. Contributing authors are not limited to neurosurgeons, but also include neurologists, physiatrists, endocrinologists, and more, primarily affiliated with Thomas Jefferson University with additional outside collaborators from Stanford University and University of Pittsburgh.
Features: The topics range from acute spinal cord injury to subarachnoid and intracranial hemorrhages in addition to the medical management of complications that may arise secondary to these pathologies, such as sodium disorders, seizures, endocrine dysfunction, and hemodynamic instability. Guidelines are supported with landmark research trials and with a plethora of footnotes, which provide readers the opportunity to review the original source in more detail. However, the number of pages dedicated to citations could be reduced by decreasing the font size or placing them at the end of the book in order to allow the content to flow more easily. Many commonly used grading scales are provided within each relevant chapter, but it may also be useful to add an appendix with these.
Assessment: This book provides an adequate summary of medical considerations in neurosurgical patients, but its language and level of detail is more on par with a handbook than scientific textbook. This book is ideal for professionals just entering the neurosurgery specialty at the more junior levels who wish to learn about the major preoperative, perioperative, and postoperative components of neurosurgical medicine.
