Purpose: The purpose is to provide guidance on how to make medications safer.
Audience: This book is written for those in an industry, retail, hospital, academia or other setting which involves medication usage. The editor is a nationally known expert on medication errors and is president of the Institute of Safe Medication Practices.
Features: This comprehensive book is unique to the field of pharmacy. It provides insights into the causes of dispensing and prescribing medication errors. The editor and contributing authors use actual examples and photos which depict how errors can be easily made if caution is not exercised. Of particular use are the numerous tables, color plates, and figures. A detailed chapter on "high alert" medications is a must-read chapter for anyone involved in medication prescribing or dispensing.
Assessment: This book is unique in the field. It should be a mandatory read for all pharmacy students and is certainly a valuable addition to any academic, industrial, or personal library.

This book provides a comprehensive overview of the types of medication errors, processes that lead to errors, research on medication errors, and suggestions for minimizing the potential for future errors. The suggestions provided for minimizing the occurrence of medication errors focus on improving existing systems rather than employee practices. The purpose is twofold: to provide a background on medication errors and highlight some major errors in the past, as well as educate readers on methods to minimize the potential for future errors. The target audience includes all healthcare professionals who are involved in any way in the process of getting medications to patients. This includes pharmacists, nurses, physicians, and pharmacy technicians, as well as administrators who supervise reporting of adverse drug events and who establish medication use procedures in healthcare institutions. This book is organized systematically and is easy to use. Related chapters are further organized into parts, and there is an index. There are color and black-and-white photographs of products that have been associated with medication errors, and there are samples of unclear medication orders. These photographs and samples enhance the text and further demonstrate the author's point. The chapters are referenced. This is an informative book. It is very thorough and provides valuable suggestions to practitioners for preventing medication errors. All healthcare professionals involved in the medication use process will benefit from reading this book. Healthcare administrators should consider this information while establishing medication use processes.
