Overview

Remarkable progress in the development of new concepts and techniques used in reconstructive surgery of microtia/atresia of the external auditory canal (EAC) has been made since the beginning of the 21st century. Helical computed tomography has made a three-dimensional reconstruction of the soft tissue of the temporal bone surface and the cranium possible, and has laid the groundwork for a collaboration between plastic surgeons and otologists. This book presents the latest findings on reconstructive surgery performed jointly by plastic surgeons and otologists. Based on this concept, information on diagnosis, surgical procedures, outcomes, long-term results and psychology is discussed. Collaborative surgery offers advantages not only in terms of a better reconstruction of morphology and function, but also in terms of the lower number of surgical procedures required which reduces the psychological pressure and economic burden on patients.

Description: This fairly comprehensive review of microtia and aural atresia, from genetics to surgical steps, emphasizes a combination microtia and aural atresia procedure.
Purpose: The authors' aim is to describe their techniques for improving the cosmetic appearance of the ear as well as the hearing with a single surgery. This is an interesting description of this technique, which is described well.
Audience: The book is useful for all practitioners, particularly otolaryngologists and plastic surgeons, looking for advanced microtia/aural atresia techniques.
Features: It covers a broad range of topics related to microtia and aural atresia, and does a good job discussing the problem outside of surgical technique as well. This includes modern genetic syndrome, classification of ear anomalies, the psychological impact of microtia, and the hearing and learning problems associated with aural atresia. The book extensively describes the authors' technique for microtia repair, which is combined with aural atresia repair, in what is claimed to be a six-hour operation. There are numerous drawings and high-quality photographs. However, some of the chapters are quite brief (complications such as skin flap necrosis), and it would be helpful to have more extensive descriptions. Also, the book focuses on the authors' technique, which is based on Nagata, and Brent and Nagata techniques are discussed briefly. It is unfortunate that there is no description of Firmin's technique or classification of ear anomalies with a corresponding algorithm for management.
Assessment: Overall, this is a useful resource for surgeons and trainees working with and operating on microtia patients. The combined technique is useful. But this is one group's opinion of a technique, and leaves out important topics such as the Firmin technique. Also missing is an extensive discussion of osseointegrated hearing devices, which have been shown to be very useful in patients with aural atresia.
