Description: This is the third edition of a book that emphasizes the physiology of organ systems affected by surgical problems and procedures. The first edition was published in 1988 and the second edition in 1998.
Purpose: Surgical disease is presented in the context of physiologic changes.
Audience: Students and residents with a broad interest in surgical disease will find this book attractive. Editors and authors are American authorities in surgery and represent leading medical schools in North America.
Features: Initial chapters cover the general management of the surgical patient with consideration of infection, bleeding disorders, shock, neoplasia and anesthesia. A second group of chapters discuss surgical physiology by organ system proceeding first through the gastrointestinal tract followed by the cardiovascular, nervous, peripheral vascular, and endocrine systems. Wound healing and cutaneous considerations are discussed followed by reviews of physiologic changes at the extremes of life. Chapters are written in detail and contain extensive reference lists dating to within two to three years of publication. Black-and-white line drawings, photographs, and radiographs appear throughout, but print quality could be better in some of the figures. The table of contents provides chapters in appropriately organized groups, gives authorship, and provides subheadings for each presentation. An extensive subject index of over 20 pages concludes the book.
Assessment: I agree with the observation made by Dr. Miller in his preface that the approach to surgical disorders is frequently based on memorized schemes or protocols rather than thorough understanding of pathophysiology. This book is an appropriate tool to address this problem. While this is a complementary rather than a standalone text, I enjoyed reading it. It will be of particular service to those working in preparation for in-training and board examinations.

This second edition is a fairly complete updating of the encyclopedic text first published in 1988. It is devoted to integrated system physiology and a description of specific organs. It intends "to bridge the gap between basic science physiology and physiologic derangements responsible for surgical diseases." The book is intended for general surgeons and will be most useful for general surgery residents in training. The format assumes familiarity with surgical practice, and therefore is not suitable for medical students. It does provide a valuable review of basic science principles for the practicing surgeon. In the organ specific chapters, the text focuses on pathophysiology, but most of the illustrations and diagrams describe elementary anatomy and diagrams of surgical operations with the ultimate result that coverage of both topics is sometimes superficial. Many of the illustrations are reproductions from old textbooks which sometimes give the book a patchwork character. Nonetheless, the descriptions of basic physiology and pathophysiology are excellent with minimal duplication across chapters. The discussions of coagulation, GI physiology, and renal physiology are outstanding. The new title is misleading. A resident would have difficulty managing mechanical ventilation, a major liver injury, or obstructing colon cancer based on this book alone. As an exposition of "the physiologic basis of surgical disease," however, it is an excellent addition to any surgical resident's library.
