Overview

A triaxial system (Axis I—Clinical diagnoses, Axis II—Disabilities, Axis III—Contextual factors) is intended for use in clinical, educational and research activities. It aims to ensure that disabilities and factors relevant to the management of a mentally ill patient's condition are systematically recorded. It has been prepared and trialed by an international team of experts and has proven easy to use, helpful and applicable to a wide range of cultures and settings. This comprehensive manual provides information on the development of the ICD-10 multiaxial system and describes its structure and use. It details ICD-10 categories for each axis of the system as well as instruments and instructions for their application. The instruments include the ICD-10 multiaxial diagnostic formulation form, WHO short disability assessment schedule (SHO DAS-S) and Axis III checklist.

Description: This is an adaptation specifically for adult psychiatry of the tenth revision of the International Statistical Classification of Diseases and Related Health Problems, published by the World Health Organization. It is one of several versions of the ICD-10 classification of Mental and Behavioral Disorders targeted at specific users. Other versions are available for use in primary care and research settings. The multiaxial system described in this volume was tested for cross-cultural applicability and reliability in field trials involving 274 clinicians from 20 countries spanning all regions of the world.
Purpose: This book is designed as a tool for clinicians' multiaspect assessment and comprehensive formulation of the psychiatric patient's clinical condition.
Audience: The targeted audience is psychiatrists in general psychiatric practice settings.
Features: The book consists of two parts. Part I contains a useful introduction, brief historical background, and descriptions of the three axes comprising the multiaxial system: clinical diagnosis, disabilities, and contextual factors. Part II contains a glossary of clinical diagnoses, the WHO Short Disability Assessment Schedule, and a listing and definitions of contextual factors (referred to as "Z factors" in ICD-10 nomenclature).
Assessment: This book succeeds as a quick and easy introduction to the ICD-10 multiaxial system. Assessment and rating of disability and contextual factors are particularly helpful and straightforward. However, no attempt is made to compare and contrast the ICD-10 with the DSM-IV, and American psychiatrists will find much to quibble about. Still, no one will disagree with the need for a classification system that allows us to communicate more easily with our international colleagues.
