Description: This pocket guide will help students and nurses across all healthcare settings identify nursing diagnoses, priority-based nursing interventions, and associated rationales. It is a valuable resource for nursing students learning to write care plans. The diagnoses contained in this guide, are consistent with the suggested NANDA-I list. The previous edition was published in 2016.
Purpose: The purpose is to provide a comprehensive reference that will assist nurses, nursing students, or research nurses in identifying priority-based interventions associated with specific nursing diagnoses as proposed by NANDA International. This guide is meant to facilitate and promote the evaluation and documentation of patient care through use of a standardized language (i.e. ICD or SNOMED). This allows for uniformity in computer coding and reimbursement across all healthcare settings and seamless continuity of client care among multiple healthcare providers. This is a comprehensive guide to nursing diagnoses, priority interventions with associated rationales, desired outcomes, evaluation criteria, and documentation using the nursing process as its foundational underpinning.
