Description: This pocket-sized reference will be helpful as a clinical reference guide in obstetrics and pediatrics. Tabs on the bottom delineate sections, and there is an index to allow for easy lookup of specific topics. This is the fourth edition. The third edition was published in 2017.
Purpose: The purpose of this book is to allow for easy access to tables/diagrams and lists of topics/education when caring for pediatric and obstetric patients. This is needed by anyone new to these specialties, or students rotating through them. The objectives are met by providing the most salient topics needed.
Audience: This book is appropriate for nursing students prior to their pediatric or obstetric/postpartum rotations. It would also be helpful to nurses new to any of these specialty areas. Overall, this book meets the needs of the audience. The authors are a variety of nurse practitioners working in these specialties.
Features: This book covers the specialties of obstetrics, gynecology, and pediatric care. It is broken into subsections such as pediatric basics or pediatric acute care as well as intrapartum and postpartum care to allow for easy access and retrieval of information. Each section begins with many of the commonly used acronyms or terms used in that specialty, which is helpful. Throughout most of the book, reference is made to "primary healthcare providers" instead of solely physicians, midwives, or nurse practitioners, which is inclusive of all providers nurses will work with within these specialties. The most important aspects of caring for patients in these specialties are noted in charts, pictures, or bullet points. This makes for easy access and retrieval while actually caring for patients. The book is also waterproof, which is useful on the job. The diagrams are excellent. In the intrapartum section, there are a couple of things that are unnecessary. Fundal height does not need to be measured at that point. There also does not need to be cleansing of the perineum. There are a couple of inaccurate notes. For example, the turtle sign occurs after the birth of the head. Lightening can happen two or more weeks before labor. It is also noted that the cord should be clamped 30 to 60 seconds after birth, which is not in the best interest of most newborns and should not be done until pulsations stop. There is also mention of helping patients ambulate post-epidural. It is unclear if they mean after labor is over, or after the catheter is placed. Some patients with epidurals will have limited or no mobility and the wide range of movement possibilities should be addressed. The basic pediatric section notes that mothers of newborns should be assessed for postpartum depression at all the newborn visits to six months, but it should state these checks occur until one year. More information is needed about how PROM is defined, not just what it stands for. There could also have been more on when it is appropriate to do a vaginal exam and when it is not. There were also some areas that needed more information. There was no mention of assessing the newborn on the maternal abdomen and how stabilization could occur there. There was nothing about immediate management of postpartum hemorrhage and what the nurse would need to do to handle that complication. More information on comfort measures during labor, as well as patient education during that time, might also be helpful.
Assessment: I would recommend this book for use as it is very thorough and accurate. I also like the ease of use in answering specific clinical questions someone might have when providing care. The information that is inaccurate or needs clarification is not so detrimental to prohibit its use in practice. Information has changed in the five years since the third edition, so a fourth edition was needed to keep current with immunizations, medications, treatment plans, and evidence-based information.
