About the Author

​Daniel Joseph Meara, MS, MD, DMD, FACS, FICD, is Attending Faculty at Christiana Care Health System, Oral and Maxillofacial Surgery Residency Program, Department of Oral and Maxillofacial Surgery in Wilmington, DE. Dr. Meara is Chair of the Department of Oral and Maxillofacial Surgery&Hospital Dentistry at Christiana Care Health System and also an Affiliate Assistant Professor in the Department of Physical Therapy at the College of Health Sciences, University of Delaware. He gained his DMD from the University of Alabama in 2005 and completed an Oral and Maxillofacial Surgery Residency at that university and a Craniomaxillofacial Surgery Fellowship at West Virginia University/Charleston Area Medical Center. Dr. Meara is a Diplomate of the American Board of Oral and Maxillofacial Surgery, a board member of The Mid-Atlantic Society of Oral and Maxillofacial Surgeons, and President of the Delaware Academy of Medicine/Delaware Public Health Association. In addition to his clinical practice, he is active in research and has published almost 30 original journal articles. 
Raj Gutta, DDS, MS, is a board-certified Consultant in Oral&Maxillofacial Surgery at the Midland Memorial, Midland, TX, and the Medical Center Hospital, Odessa, TX. He is an AOCMF Craniomaxillofacial Faculty Member (North America) and a Fellow of the American Association of Craniomaxillofacial Surgeons (AACMFS) and the American Association of Oral and Maxillofacial Surgeons (AAOMS). Dr. Gutta completed his training in oral and maxillofacial surgery at the University of Alabama at Birmingham and also obtained his Master’s degree from that university, where he was awarded the School of Medicine Fellowship Award. He has served on faculty positions at the University of Texas Health Science Center in San Antonio, the University of Cincinnati, and the Cincinnati Children’s Hospital. Dr. Gutta practices a full range of oral and maxillofacial surgery and also offers a wide range of treatment including routine oral surgery, dental implants, orthognathic surgery, and TMJ surgery. He acts as a reviewer for leading journals in the field and has been the recipient of several international awards.

Description: This book covers management of immunocompromised patients within the realm of oral and maxillofacial surgery. Specifically, the book discusses how clinical manifestations and typical medications of these patients affect surgical outcomes from an oral surgery standpoint. It also outlines possible post-operative complications and proper preventive measures.
Purpose: The purpose of this book is to outline the care of immunocompromised patients and discuss the pathophysiology of their systemic condition and management techniques from the OMFS perspective. These worthy objectives are met and the book is needed. The book offers specific guidelines that are useful in daily clinic settings. For example, the book presents guidelines on prophylactic antibiotics in patients with rheumatoid arthritis. Although antibiotics are not generally recommended, the guidelines emphasize, at the very least, having a discussion with the orthopedic surgeon regarding specific indications for antibiotic coverage, which may be prudent.
Audience: The book is written for students, residents, and oral surgery practitioners and it meets the needs of the intended audience. The authors are credible as recognized faculty and private practice OMFS specialists.
Features: The book's best aspects are the coverage of specific systemic diseases, their respective pathophysiology, and how to manage them accordingly. There are numerous cut-offs explained for managing patients in the oral surgery setting with systemic disease. For example, there is a subsection on HIV that displays numeric counts for treatment in regard to CD4+ count and whether these patients need alteration to standard surgical techniques and prophylaxis to decrease chances of postoperative surgical site infection. A shortcoming of the book, however, is the lack of illustrations and clinical imaging, especially in comparison to the number of bullet points and tables provided to demonstrate protocols of management. For example, when discussing postoperative complications in patients with cancer, clinical images of the complications would have been useful. When discussing clinical features of systemic diseases, often times a bulleted list will state the manifestations of said systemic disease, but added clinical images would help captivate the attention of the audience and allow for better retention.
Assessment: This comprehensive book offers a guide to systemic diseases and their respective pathophysiology and management. One of the book's hallmarks is its attention to detail on clinical manifestations of each systemic disease. For example, when discussing adrenal cortical hyperfunction, each specific clinical hallmark for Cushing syndrome is discussed as well as how to manage patients - for example, whether specialty consultation should be considered in addition to prophylaxis dosing. Furthermore, the book effectively outlines how the clinical manifestations affect patient management from an oral surgery perspective and alterations in techniques that can lessen the chances of postoperative complications.
