Description: This book covers orthopedic - bony and soft tissue - injuries encountered in the emergency department. It is divided into four parts, on general principles, spine, upper, and lower extremities. Each chapter includes a detailed discussion of specific injuries and their management. In its eighth edition, this version includes new clinical photos and imaging, videos, and a more extensive discussion of musculoskeletal ultrasound.
Purpose: The purpose, according to the authors, is to provide practitioners with a concise resource covering "everything they need to know" to manage patients with orthopedic injuries. They describe common mechanisms of injury, recommendations for imaging and treatment, and potential complications seen in an emergency department setting. In the first edition of this book published in 1982, the authors noted that although orthopedic injuries accounted for a large portion of complaints emergency physicians see daily and in a more acute setting than when orthopedic surgeons typically evaluate, available publications were either directed toward surgeons or junior medical students. Their goal was to provide emergency physicians with a more comprehensive and clinically useful book. Now in its eighth edition, the authors' original objective remains a worthy one. This is a relatively succinct yet thorough review of injuries that can be easily accessed and used in the clinical setting.
Audience: This book was written by and for emergency physicians. The last two editions featured chapters authored by clinicians with a particular interest or expertise in the topic. Contributors primarily practice in the field of emergency medicine and, thus, are uniquely positioned to create a reference useful for the assessment and management of acute orthopedic injuries. These authors hail from well-respected emergency departments and residency programs across the country, further ensuring that the book is both accurate and applicable to physicians practicing in a variety of settings and locations.
Features: The first part contains a useful fracture index with drawings of specific types of fractures organized by anatomic area. The main body of the book is divided into two parts - general orthopedic principles and management and injuries. The second part is further divided by anatomic area covering spine, upper extremity, and lower extremity. Part I contains particularly useful reviews of analgesia (e.g. nerve blocks), rheumatological processes (e.g. aspiration to evaluate for gout and septic joints), special imaging and ultrasound techniques, and considerations specific to pediatrics. Each chapter in parts II-IV includes a discussion of relevant anatomy and common mechanisms of injury, recommendations for evaluation, associated injuries, and management principles. Readers are likely to find the diagrams, radiographs, and clinical photographs especially useful in augmenting the text descriptions of injuries. Part V, the appendix, contains a very brief review and diagrams for specific splinting techniques that are referenced in the book.
Assessment: This comprehensive book covers orthopedic injuries in a concise fashion that physicians practicing in a busy emergency department will likely find useful. The unique fracture index provides figures of fractured bones, enabling clinicians to visualize specific injuries, then go directly to the location in the book where pertinent information is provided. This feature, in conjunction with the organization into anatomic areas and the authors' attention to information pertinent to evaluation and management in an acute setting, help make this book especially useful. The eighth edition features a more thorough discussion of ultrasound techniques, updated clinical images, and additional multimedia resources than previous editions. Given the prevalence and incredible diversity of orthopedic injuries seen in most emergency departments, this book will be very useful to physicians in an acute setting.
