Overview

Over the past 5 years there has been great excitement and controversy in the scientific, financial, and lay literature for the potential of stem cell-based strategies for the prev- tion and treatment of chronic heart failure (CHF). Not that long ago we believed we were born with a set number of cardiac myocytes and that once damaged there was no hope to replace them. The interest in the field stems from the magnitude of cardiovascular disease in the world. Our ability to treat and help patients survive acute myocardial infarction (MI) has resulted in a near epidemic of CHF. There are more than 5 million Americans who currently carry the diagnosis of CHF. With more than 1 million MIs a year in the United States, there are approx 500,000 new cases of CHF diagnosed each year. The goal of Stem Cells and Myocardial Regeneration is to present, in a coherent manner, the current state of knowledge of stem cell-based therapies for cardiac dysfunction, including current findings in both the laboratory and the clinic trials. The first section of this Stem Cells and Myocardial Regeneration focuses on the magnitude of the problem and the successes and failures of what we consider optimal medical therapy. It is on this background that stem cell-based therapy needs to build.

Description: This multiauthored book focuses on stem cell therapies for cardiac repair.
Purpose: It presents the current knowledge in stem-cell based therapies for cardiac dysfunction, from bench to bedside including recent clinical trials.
Audience: The intended audience includes cardiovascular scientists, physiologists, and physicians such as interventional cardiologists and heart failure specialists.
Features: The first of the four major sections focuses on cells of interest for myocardial regeneration. It is subdivided into eight chapters which individually discuss hematopoietic stem cells, mesenchymal stem cells, multipotent adult progenitor cells, mesenchymal progenitor cells, umbilical cord blood stem cells, endogenous cardiac stem cells, and embryonic stem cells in relation to myocardial repair and regeneration, and angiogenesis. The second section focuses on the mechanisms and critical pathways involved in myocardial repair with chapters covering chemokine and homing factor expression, stem cell differentiation, and arrhythmogenic risk of cell therapies. The third section relates strategies for cell delivery with chapters dedicated to percutaneous and open chest techniques. Section four discusses clinical trials regarding stem cell therapy for cardiac dysfunction, including those on whole bone marrow transplantation, clinical angioblast therapy, and use of skeletal myoblasts for treatment of chronic heart failure. The summary at the beginning of each chapter provides a nice synopsis and is a strength of the book.
Assessment: The authors have succeeded in their objectives with a systematic presentation of the individual cells of interest for myocardial regeneration, the mechanisms and critical pathways involved in myocardial repair, the strategies for cell delivery, and a review of pertinent clinical trials. This is a useful reference for those interested in stem cell biology as a treatment for ischemic heart disease and chronic heart failure.
