Around the time of writing the first edition – about a decade ago – there were very few monographs on this s- ject: today, there are possibly no less than 20. Based on critical inputs, this edition stands thoroughly revamped. New chapters on ventilator waveforms, airway humidification, and aerosol therapy in the ICU now find a place. Novel software-based modes of ventilation have been included. Ventilator-associated pneumonia has been se- rated into a new chapter. Many new diagrams and algorithms have been added. As in the previous edition, considerable energy has been spent in presenting the material in a reader-friendly, conv- sational style. And as before, the book remains firmly rooted in physiology. My thanks are due to Madhu Reddy, Director of Universities Press – formerly a professional associate and now a friend, P. Sudhir, my tireless Pulmonary Function Lab technician who found the time to type the bits and pieces of this manuscript in between patients, A. Description: This is the second edition of a single author monograph describing management of mechanical ventilation.
Purpose: The relationship of mechanical ventilation to classic medical physiology is emphasized.
Audience: Trainees in critical care medicine, physicians providing coverage in the intensive care unit, and teachers of critical care medicine may find this well-written book beneficial.
Features: After a brief review of the history of mechanical ventilation, the book describes conventional modes, physiologic effects, alarms settings, physiologic monitors, and treatment strategies for specific disorders. Chapters are clearly written and break difficult concepts down into understandable pieces. The book makes ample use of diagrams and tables of excellent pedagogic and print quality. Chapters include short reference lists featuring original work and consensus statements. Although some references date from before 2000, this is not a significant concern, because they are consensus statements and fundamental papers. The table of contents describes chapters and key subheadings. Twelve case studies describe the approach to common ventilator problems, and a subject index of 20 pages provides adequate access to content.
Assessment: This second edition is extensively revised from the first edition, published approximately a decade ago, and I appreciate the new chapter on monitoring waveforms. Finally, I offer two observations. First, a minor point is that there is a relatively large number of typographical errors. Second, the use of mechanical ventilation entails art and science, and not all practitioners will agree with the "art of mechanical ventilation" as described here.
